Welcome prospective Family Flex members!  At this point of the process you have filled out the Wait List Form and have been notified of a start date by Family Flex.

Please contact Family Flex for an enrollment form.  These forms are only for children who will begin in a full-time or part-time program.  Please plan on 1.5 hours to fill out this information that includes medical, family and personal history of your child.  Family Flex can either fax or e-mail the enrollment forms to you once your child has a prespective start date.  It is recommended that you print the form and fill it in manually and submit the forms with all the appropriate attachments as requested. 

Please note that your child cannot attend their first day until all of these forms are completed.  Thank you.
